Abstract

National crises frequently expose weaknesses in governance systems, public administration, security institutions, political leadership, legal frameworks, and accountability mechanisms. In the aftermath of major failures, governments often establish commissions of inquiry to investigate causes, assign responsibility, restore public confidence, and recommend reforms. These commissions occupy a unique position within modern governance systems because they operate at the intersection of accountability, transparency, institutional learning, public trust, and policy reform.
